HOME AND COMMUNITY FACILITATION
ACADIA's Home and Community Facilitation Program is designed to provide one-to-one individualized and goal-oriented intervention in the individual's home and community. Services focus on teaching the individual and/or primary care-giver strategies that will enhance his/her quality of life, and improve independence, adaptation and self worth.

The intensity and frequency of services are geared to each individual's needs. As independent or adaptive skills increase, therapeutic interventions decrease. Because ACADIA provides a continuum of services, individuals can access members of the ACADIA transdisciplinary team of specialists as needed. Areas such as vocational development, socialization and leisure, activities of daily living, behavioral management, cognitive adaptations, transportation and stress management can be addressed fully. All services are provided one-to-one in the individual's home and community.
